Dear Athletes, Parents, Guardians, and Caregivers:
Special Olympics Indiana is making several changes for all traditional athletes. One change is a new online Registration and Medical Form. The Registration and Health History Forms are completed online, and the Physical Examination Form must be completed and signed by a licensed medical professional. This is mandatory for all new and existing traditional athletes. All current medicals EXPIRED December 31, 2021.
First, a new four-page Registration must be completed. This is completely done online. The next form to be completed is a two-page Health History about the athlete. Again, this needs to be completed online. The last form is a two-page blank Physical Examination Form. This form needs to be filled out and signed by a licensed medical professional.
Once the Registration and Health History Forms are completed online, all eight pages must be printed. Please keep the Registration and Health History pages. You will turn them in later. Please take the blank Physical Examination Forms to a licensed medical professional to be completed and signed. Once the Medical Form is signed, turn in all forms to Kevin Anderson.

The forms take about fifteen to twenty minutes to complete. Once you start, you should finish the whole thing. Otherwise, you will lose all information you already put in, as it does not save anything.
